Celebrating National PACE Month

Please join Cognisight and Programs of All-Inclusive Care for the Elderly (PACE®) across the country, to celebrate National PACE Month in September. PACE programs coordinate and provide preventive, primary, acute and long term care  services, to nursing home-eligible seniors, so they can continue to live independently within their communities as long as possible, while receiving care in the most cost-effective way. This year’s theme, “Many Hands, Many Minds, Many Goals,” celebrates the roles the interdisciplinary team plays in promoting independence for PACE participants. As a member of the National PACE Association (NPA), Cognisight has been a long-time supporter of the PACE model and provides risk adjustment services to their programs throughout the country.
